Position Summary: The Admissions Health Care Coordinator will be responsible for census development, community relations and community education with both existing and new referral sources for the facility. This position will receive and review admission information for the facility from various referral sources. Attention to detail, critical thinking, responding in a timely and professional manner and teamwork are essential to this role. This position will assess, have a good knowledge of, and sensitivity to, the needs, services and care coordination of, and for, their clients, residents or patients. The position is responsible for developing and executing a strategy to maintain and increase referrals and provide distinctive customer service to new and existing customers. The position will also work with the regional support to develop and produce appropriate collateral materials as needed. Job Duties: This position is responsible for facilitating and coordinating the admission process and discharge process with residents and family members. Additionally, utilizing marketing materials, advertising, and public relations to promote the building and increase census will be part of the daily responsibilities. This position will be a vital member of the campus leadership team and is expected to participate in strategic planning for the campus.


JOB REQUIREMENTS
Qualifications: Must have a degree in social services, nursing (LPN/RN, Medical records) or in related field (marketing, sales or equivalent experience); strong aptitude in communication - writing, spelling, reading and articulation of speech; ability to multi task, prioritize and manage time. Show high degree of initiative, strong organizational and follow through skills. Be computer proficient in Word, Publisher, and other Microsoft applications. Must possess strong customer service skills; be friendly, flexible, enthusiastic, and outgoing. Must have experience in healthcare, admissions, marketing and sales, senior service experience preferred; must have a current unencumbered driver's license and ability to travel in the community by car. Must be able to perform essential functions of the position with or without reasonable accommodation.
