This position will travel or "float" to both The Estates at Linden facility and The Estates at Greeley facility. Both facilities are located in Stillwater, Minnesota. This is a full time position. We value their contributions and work hard to attract and retain the best talent by offering a positive and supportive work environment along with competitive pay, nice perks, and great benefits A Day in the Life as a Minimum Data Set (MDS) Coordinator As an MDS Coordinator, you arrive each day determined to ensure that the residents' physical, mental, and emotional needs are truly met, while also ensuring that the federally-mandated process for clinical assessments is properly followed for each resident. You continuously monitor, evaluate, and manage the care given. You oversee the timely and accurate completion, editing, locking, and transmission of all MDS assessments. Due to your excellent and in-depth knowledge of the MDS process and your heart for and understanding of the people we serve, you are able to balance regulatory compliance with meeting the real needs of real people. You perform the initial interview and assess each new resident. You complete the MDS assessment based on your direct observations, review of their patient records, use of a data collection worksheet, interdisciplinary team data, and interviews with residents, families, and staff. From this assessment and evaluation, you develop a care plan that has long-term and short-term goals for improvement. You conduct meetings with the multidisciplinary treatment team, caretakers, and families to discuss these goals. Sometimes, you also reach out to specialists within the community to get help in achieving care plan goals. You help develop the MDS completion schedule, setting the assessment reference dates, and care conference dates. You prepare and electronically transmit timely reports to the national Medicare and Medicaid database. Your attention to the accuracy of medical records and coding procedures is essential to the receipt of proper reimbursement and the ability of our facility to continue to operate and provide the highest quality of care to our residents. You are also an advocate of residents' rights and ensure that each resident and their family are aware of their legal rights. You support the nursing staff in providing accurate assessments, care plans, and excellent quality of care. You provide direct observation and participate in resident care in order to monitor the residents' needs and staff interactions with the goal of increasing MDS coding accuracy. JOB REQUIREMENTS
Qualifications * Current registration as a registered nurse (RN) in the state of Minnesota * Current certification in CPR and emergency care for the healthcare provider * Must be able to perform essential functions of the position with or without reasonable accommodation. Must qualify for employment, after criminal background study, per guidelines of Minnesota Department of Human Services. Experience with Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) is preferred, but not required. Do you have excellent communication skills, both verbal and written?
