Infection Preventionist

This position is located in Two Harbors, Minnesota at The Waterview Shores. POSITION SUMMARY: The Infection Preventionist oversees, facilitates and manages infection control and prevention and antibiotic stewardship practices at the facility. This position includes infection control, which will be designated to half of this role. The Infection preventionist will track and trend all infection and serve as the infection preventionist role. They will ensure that the nursing staff is knowledgeable when it comes to documentation and following the infection process and ABT stewardship. This person will retrain when there is any evidence of spread from infection. The position is responsible for surveillance, maintaining records, data collection, and assessment, evaluation of Incident Reports, reporting to Safety Committee, Infection Control Committee, and Quality Assessment/Assurance Committee. This position will also included Minimum Data Set Coordinator duties. As an MDS Coordinator, you arrive each day determined to ensure that the residents' physical, mental, and emotional needs are truly met, while also ensuring that the federally-mandated process for clinical assessments is properly followed for each resident. You continuously monitor, evaluate, and manage the care given. You oversee the timely and accurate completion, editing, locking, and transmission of all MDS assessments. Due to your excellent and in-depth knowledge of the MDS process and your heart for and understanding of the people we serve, you are able to balance regulatory compliance with meeting the real needs of real people. You perform the initial interview and assess each new resident. You complete the MDS assessment based on your direct observations, review of their patient records, use of a data collection worksheet, interdisciplinary team data, and interviews with residents, families, and staff. From this assessment and evaluation, you develop a care plan that has long-term and short-term goals for improvement. You conduct meetings with the multidisciplinary treatment team, caretakers, and families to discuss these goals. Sometimes, you also reach out to specialists within the community to get help in achieving care plan goals.


JOB REQUIREMENTS
QUALIFICATIONS: Must be a Registered Nurse, currently licensed by the State of Minnesota, who has the Infection Preventionist training/certifications as outlined by the CDC in collaboration with CMS, and preferably quality assurance, gerontological, or rehabilitation nursing experience. Must have an interest in and knowledge pertaining to, microorganisms, infection, prevention, antibiotic stewardship, continuous quality improvement, and quality measures. Must have an interest in teaching both professional and non-professional staff. Must have basic computer competencies or be willing to attend training to attain competency. Must be flexible with working hours and possesses the ability to work independently. Must be able to perform essential functions of the position with or without reasonable accommodation.
