THIS POSITION IS LOCATED AT PARMLY ON THE LAKE IN CHISAGO CITY, MN Position Summary: Social services interacts with the residents, families, visitors, other facility staff, and community agencies during the admission and discharge process. They are responsible to market the services we provide, and establish relationships with all referral sources. They clarify a range of legal documents for residents and families as well. As a member of the interdisciplinary care planning team, the Social Service position identifies the psychosocial strengths and needs of each resident, assists in developing an individualized plan of care, and provides medically-related social services to help the resident attain or maintain the highest level of functioning possible. Job Duties: This position will assist with facilitating and coordinating the admission and discharge process. Make referrals to home health agencies as requested. Accurately maintain resident records. Participate in the interdisciplinary patient care planning team. Visit residents individually to help assess resident needs. Communicate efficiently with staff, residents, family members, and community members. Must maintain knowledge of community resources for the residents. Qualifications: Must recognize and understand the psychosocial and emotional needs of residents. Position requires strong skills in communication, organization, management and leadership. Must be able to perform essential functions of the position with or without reasonable accommodation.
