RN MDS Coordinator

Seasons Healthcare of Trimont is looking for an RN MDS Coordinator to join our team. The primary responsibility of this position is to guide the assessment and treatment process of residents. This includes completion of the nursing care plan, MDS, and attending care conferences and picking up 1 shift per week. The RN MDS Coordinator manages the Restorative Nursing Program along with assisting the Charge Nurse and DON. This position is Full-time with benefits and has flexible hours. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Qualifications: • Must have a current licensure as a Registered Nurse in the state of Minnesota • Have thorough knowledge of Federal and State guidelines, Medicare and Case Mix reimbursement or the willingness to obtain training in these areas • Current CPR/BLS certification • Have knowledge of gerontological nursing and related aspects appropriate to resident population • Must possess leadership skills and can make sound decisions and work under pressure • Skilled in oral and written communication • Self-driven and detail-oriented to complete resident assessments accurately and by the deadline. • Experience in completing MDS and care plans preferred or the willingness to obtain training in these areas • Capable of working closely with other departments as well as the facility providers
JOB REQUIREMENTS
Job Duties/Responsibilities: • Coordinate, develop, monitor, and maintain current resident assessments as required • MDS- initial, quarterly, annual and change in condition • Resident care plans-assure that initial temporary care plan is initiate upon admission, write master care plan upon completion of MDS, prepares quarterly summaries and attend Care Conferences • Assure that the highest Case Mix/Rug level is being obtained • Transmit MDS by deadline, retrieve Cas Mix and Validation reports, validate for correct/completed submission, and distribute to appropriate departments • Initiate and maintain all Medicare and MSHO documentation, tracking and communication upon resident admission or qualifying event. Along with recertification forms. • Attends and participates in necessary meetings (QA, Medicare, Stand-up, Leadership) • Round weekly with the physician/NP, enter orders and ensure follow up with families with any changes • Maintain monthly, quarterly, and annual labs for residents • Train new employees on MDS charting and its importance • Assist the DON with new admissions
