The Facility Finder can help you find a long-term care facility/housing for your loved one.
You can specify a search within a certain zip code radius, by city, or by facility.
To begin your search, type in your zip code and choose the distance (any, 5, 10, 25, etc.) or choose Search by city or facility, and then click on Search.
A list of long-term care providers will appear, along with individual links for more information and a location map. Click on these links and you’ll be redirected to the provider’s website where you can find up-to-date information and contacts.
