Position available on PM shift position 230pm-11pm and NOC shift: 1030pm- 630am. Assists RN in responsibility of total resident care for apartment and assisted living residents. Performs professional nursing duties in accordance with agency policy and in accordance with state home health regulations. Assists in monitoring the home health aide's performance. Must be pleasant, respectful and courteous to residents, families, visitors and fellow employees at all times. MAJOR JOB FUNCTIONS: Provides nursing care to apartment and assisted living residents. ­ Sets up and monitors medications of resident as ordered by physician. ­ Performs treatments as ordered by physician. ­ Assists RN in developing plan of care from service agreement and revises as needed. ­ Assigns home health aide in assisted living in accordance with plan of care. ­ Assists home health aide with assignments as necessary. ­ Completes appropriate documentation, resident concerns and routine charting. ­ Assists physician and RN in performing specialized procedures. ­ Prepares equipment and materials assuring aseptic techniques. ­ Assists in orientation of new employees. ­ Reports resident condition change to RN. ­ Delegates nursing tasks to staff who are competent to perform the procedures that have been delegated. Monitors staff. ­ Observes and monitors care given by home health aides. ­ Reports to RN on the performance of home health aides for purpose of evaluation. ­ Assists in orientation of new employees. Reports and records resident information. ­ Alerts physicians of resident changes and concerns after consulting with RN. ­ Receives and processes physician's orders. ­ Attends family conferences as indicated. -Conducts miscellaneous duties. ­ Reviews admissions, processes physician's orders and sets up medication as ordered. ­ Knows and practices good safety habits. ­ Maintains confidentiality of resident information. ­ Performs all other duties as assigned.


JOB REQUIREMENTS
JOB SKILLS, KNOWLEDGE AND ABILITIES ­ Graduate of practical nursing program for LPN and current licensure with Minnesota Board of Nursing. ­ Current CPR certification. ­ Prefer six month's experience in medical, home health or long-term care nursing. ­ Good understanding of medical, social and psychological needs of the elderly and rehab principles. ­ Must be able to read, write, and speak English. ­ Must be able to perform Physical Job Demands of position. ­ Exercises confidentiality to ensure compliance with all HIPAA regulations.
