Are you an experienced MDS nurse looking for a new role? Come join our team! New Brighton Care Center is looking for an experienced RN/MDS nurse to be a Director of MDS. New Brighton Care Center is a family owned facility, our small size allows you to truly get to know each resident. Position Overview: As the MDS Director you will be part of a multidisciplinary team. Responsibilities include MDS assessments, multidisciplinary team meetings, and maintenance of MDS schedule. You must be detail oriented, have great organizational skills, be computer literate, and have strong interpersonal skills to collaboratively work with team members and residents. Experience with online processing of assessments is a plus. As the MDS Director you will be part of a multidisciplinary team. Experience with online processing of assessments is a plus. Responsibilities: Direct the Resident Assessment Process through assisting with the completion of the Minimum Data Set (MDS) and Care Area Assessments (CAAS). Participate in developing individualized resident Care Plans, identifying the needs of the resident and projected outcomes; as required by Federal and State regulations. Rotate on-call with other nursing leaders and managers. MDS/RAI Care Plan Functions: Complete nursing assigned MDS items, CAAS and Care Plans as designated by the facility. Participate in the resident care plan meetings. Other duties as assigned by supervisors.


JOB REQUIREMENTS
Requires RN licensure in the state of MN. Experience/familiarity with QAPI, QM management, QIIP Previous experience in a hospital, long-term facility, or other healthcare related facility. Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care. Computer skills: PCC, Windows applications, Word Processing, and basic Internet knowledge. Previous MDS experience: Minimum 2 years, preferred 4 years+ Previous experience in completing MDS’ and Care Plans is required.
