The Director of Nursing will work cooperatively with the team to supervise the delivery of services to ensure the correct healthcare needs of all residents in our Assisted Living & 245D homes. Duties Include: Resident Care: Manages the health care of all residents, including the dissemination of information to families and team members. Facilitates continuity of care for those residents receiving home health care services. Communicates with resident’s physicians. Assesses residents on a scheduled and as-needed basis and records significant conditions and changes. Responsible for the nursing department 24/7, and shares on-call duties. Notifies all key individuals on each resident’s condition, as their conditions change and keeps everyone informed upon needed treatments and any significant incidents that may occur unexpectedly. Coordinates with other departments to ensure quality and proactive care is delivered consistently to each resident. Initiates a resident care plan according to the individualized needs of the resident, as prescribed by the physician and/or facility policy. Provides resident and family instruction. Stays current on all nursing care and procedures by attending educational seminars. Explains procedures and treatments to residents to gain cooperation, and understanding and alleviate apprehension. Responds to life-saving situations based on nursing standards, policies, procedures, and protocols. Leadership and Supervision: Coaches and counsel team members on individual performance expectations. Manages nursing operations and teaches team members to adhere to all expected best practices. Educates and trains team members develops ongoing training materials and focuses on team development and appropriate delegation of tasks. Supervises the maintenance of resident charts, and reviews documentation performed by caregiving team members including medication administration records. Monitors the medication given to residents. Creates solutions for all team members, residents, and family concerns. Promotes the company in a positive manner and communicates the company’s values to all audiences. Administration, Documentation, and Communication Duties: Infection Control monitoring. Review all incident reports and follow up as necessary. Review of all medication errors and team members re-training. Maintains all TB records in compliance with the MN Department of Health. Conducts pre-admission screening of prospective residents. Updates pre-admission assessment of residents as appropriate. Before and upon resident admission, complete all required documentation including nursing assessments, plans of care, medications, and other assigned forms.
JOB REQUIREMENTS
MN Registered Nurse license in good standing. Able to read, write, and speak English. Good computer software skills. Skill in recognizing the physical, mental, and emotional needs of residents. Ability to exhibit professional behavior and attitudes. Good interpersonal and communication skills. Knowledge of Federal, State, and Local regulations and nursing standards. Must be able to handle stress, and be calm under pressure. Must have a valid driver’s license. Able to respond to emergency calls, and work on call. Preferred Qualifications: Experience in working with older adults. Three years of experience in a related field.
