CARE PROVIDERS OF MINNESOTA FOUNDATION 2019 GOLF TOURNAMENT



While the day provides a wonderful opportunity for golfers to reconnect and re-energize outdoors, it also serves a vitally important function as the Care Providers of Minnesota Foundation’s major fundraising event of the year. Over the years, the Foundation has helped further the careers of hundreds of long-term care employees throughout Minnesota by providing over $467,000 in scholarships and leadership support. There’s a good chance someone you know has received a Foundation scholarship or participated in the Foundation’s Leading4Life program.

Your participation as a golfer helps ensure that we can continue to support the Foundation’s mission. We guarantee that you’ll have a great time supporting a very important cause!

Cost$175 per Care Providers of Minnesota member registrant
$200 per prospective member or non-sponsoring business partner registrant
Registration includes greens fee, cart, range balls, lunch, course games, and reception (cash bar). Golf pairings and foursome requests will be accommodated as possible, but final determinations will be based on registrations. 

 $65 of your registration fee as a member is a tax-deductible contribution; $90 of your fee as a prospective member or non-sponsoring business partner is tax-deductible.






Best ball scramble tournament. Individual prizes for closest to the pin and longest drive, men and women. Putting contest. Team prizes.



POWER TICKETS

Power Tickets may be purchased for $10 each. Each ticket is good for one entry into the prize drawings at the reception. Entire proceeds of power ticket purchases are tax-deductible.

MULLIGANS

Mulligans are back! Foursomes can purchase unlimited mulligans to be used on the course to replay a stroke without penalty. Mulligans are $20 each.

DONATE TO THE FOUNDATION

Gifts of every size will support the mission of the Foundation. If you are unable to participate in this year’s golf tournament, please consider supporting the Foundation through a direct donation using the space provided on the form inside the brochure or online.

CANCELLATION & NO-SHOW POLICY

Refunds will be issued to registrants who cancel by July 15, 2019, minus a $25 processing fee. Cancellations received after that date will be charged the full registration fee. Registrants who are unable to attend may substitute someone else in their place. No-shows will be charged the full registration fee.
