MDH and CMS will begin using a new SNF/NF survey process on November 28, 2017. This valuable binder contains all the new survey forms: (1) Survey Entrance Conference Requests, (2) Nine Required Observations, Reviews, and Interviews, and (3) 39 additional “Critical Element Pathways”. These forms, tools, and interviews replace the previous QIS forms and process. These are essential tools to help SNF/NF providers conduct self-audits and be better prepared for the new survey process. All forms are provided in the tabbed binder in both printed form and electronic form (via an included USB flash drive). A table of contents helps the user identify each specific form, tool, interview.
