Updated on January 1, 2026, to reflect the inclusion of a “designated support person,” the Minnesota Assisted Living Bill of Rights Form will satisfy the state resident notification requirements. Forms are packaged and sold in sets of 25 and are two-part carbonless with a signature page acknowledging receipt. Once signed, one copy is given to the resident while the other copy is retained by the facility to show compliance with Minnesota statute.

Assisted living facilities are required to inform current and future residents of the change to the bill of rights. The January 1, 2026 version has a signature page to prove it was received and replaces the August 1, 2022 version.

NOTE: This version does comply with 144G.90 & 144G.91
