Beginning November 2, 2024, healthcare providers, including nursing facilities and licensed assisted living facilities that provide Medicaid waivered services, are required to post a notice of non-discrimination in prominent physical locations, post on providers website, and provide upon request.

The 11 x17 poster will include your facility’s name, civil rights coordinator, and contact information in 20 point font as required.

Please note the required second step after purchase: The receipt email sent to purchasers of the poster will include a link requesting the poster’s required information. The information submitted by the purchaser will be merged with the poster and mailed to the purchaser.
