Webinar: PIPP Grants: Insight for a Successful Proposal
The latest round of Minnesota Department of Human Services (DHS) Skilled Nursing Facility/Nursing Facility (SNF/NF) Performance-Based Incentive Payment Program (PIPP) Grant Requests for Proposals (RFP) is now underway. PIPP supports provider-initiated projects aimed at improving the quality and efficiency of nursing home care.

Provider-initiated projects are selected through a competitive process and funded for up to 5% of the weighted average operating payment rate. Minnesota has made a major investment in PIPP by supporting 284 individual or collaborative projects, representing 286 facilities and total funding of over $134 million to-date.

DHS will implement the twelfth round of PIPP payments on July 1, 2018. To be considered, an organization must submit a fillable form to DHS by midnight on May 1, 2018.

Sarah is the Executive Director of Empira, a collaborative of four long-term care providers. Sarah has over 16 years of experience in health care including providing direct patient care, leadership, education, and consultation in a variety of health care settings. Sarah provides oversight to the Empira team to support innovation, exploration, diffusion, and optimization of applied research that produces clinical and holistic programs and solutions to promote quality of life for older adults. Sarah is currently enrolled in an MBA program. She has presented at state and national conferences and serves on several state association committees.


Amanda JohnsonMs. Amanda Johnson, RN, LNHA

Vice President of Clinical Operations, Tealwood Senior Living

Amanda Johnson has worked in long-term care since 1989. She holds a bachelor's degree in human services from the University of MN, Morris. She is a registered nurse, a licensed nursing home administrator, and is certified in health care compliance. At Tealwood Senior Living her responsibilities include clinical and operational oversight of skilled and assisted living communities, the development of policy and procedures, health information technology, regulatory compliance, corporate compliance, collaborative performance improvement projects, multiple aspects of quality improvement, and company leadership training programs.
