Infection Preventionist/MDS Coordinator

PRIMARY FUNCTIONS: 1. Conducts federally mandated assessments of the patients at a long-term health care facility. 2. Gather information on the facility's current patients for future assessment, including physical and mental states. 3. Assess charts and communicate with health care teams to create applicable health care plans for their current and incoming residents. 4. MDS nurses coordinate the shipment of the facility's services to its residing residents. 5. Observe and document the pricing and effectiveness of these services. 6. The MDS nurse must conduct new resident assessments, such as the Rapid Assessment Process (RAPS), to help stabilize and improve the practices of the facility. 7. Provide OBRA (Omnibus Reconciliation Act of 1987) assessments to ensure that the facility complies with Medicaid or Medicare standards. 8. The MDS nurse may be required act as the main communicator between the insurance professionals and management officials at their facility. OTHER DUITES: 1. Maintains professional effective relationships with team members, residents and resident families. 2. Works with the Director of Nursing and Administrator and performs other delegated tasks. 3. Adheres to all Nursing policies and procedures. EXPECTATIONS: 1. Understands and complies with posted work schedule. 2. Attends scheduled in- service education programs. 3. Discussed any problems, needs or concerns regarding work with direct supervisor. 4. Understands and adheres to all safety, sanitation, fire, disaster and personnel policies and procedures of the facility. 5. Performs other duties assigned by direct supervisor. 6. Follows dress code and maintains good personal hygiene. 7. Presents a neat appearance and is punctual. 8. Treats people with respect; works with integrity and ethically; upholds organizational values and mission statement; supports affirmative action and respects diversity. 9. Understands and upholds Residents’ Bill of Rights and holds all resident information confidential
JOB REQUIREMENTS
QUALIFICATIONS: 1. Must be a Registered Nurse licensed in the state of Minnesota. 2. Ability to observe and perform proper safety and sanitary procedures utilizing infection control. 3. Ability to read, write and comprehend English; Ability to follow verbal or written orders. 4. Interest in the nursing needs of the aged and the chronically ill with the ability to work with both. 5. Ability to work hours as scheduled based on the requirements of the position. 6. Ability to meet On Call requirements. 7. Detail orientated, pays close attention to detail. 8. MDS and Long-term care experience preferred. 9. AANAC, MDS credential preferred.
