RESPONSIBILITIES: • Works closely with the DON to perform initial home care patient visit and re-evaluate patient needs and progress regularly • Initiate plan of care per MD orders, DON, and CEO • Perform admission, transfer, re-certification, resumption of care, and discharges for home care patients • Educate, Monitor, Observe, Assess, and document symptoms, reactions, progress, and assessments of patient • Educate staff on home health care techniques and skills • Coordinate patient services • Assist DON to supervise LPNs and RNs onsite • Notify physician and other personnel (DON and other staff) of change(s) in patient condition • Perform skills outlined in the agency’s approved policies and procedures • Discharge patient from skilled nursing services when discharge criteria is met • Case conference with clinicians providing care to ensure coordination f care • Update clinical records according to policies and procedures • Attend in-service programs, CE programs, seminars, and self-study programs annually • Adhere to State and Federal rules and regulations. Plateau Healthcare is a growing company, founded in 2010, which specializes in private duty nursing. Plateau Healthcare is owned and operated by two RNs with extensive ICU and Emergency Medicine experience; Owners are on call 24 hours for staff. We specialize in very high complex medical clients who are on a ventilator and provide on-the-job training. We currently have an opening for a FT or PT Case Manager – MUST meet the following Qualifications, as stated below, before applying.
JOB REQUIREMENTS
QUALIFICATIONS: • Must be registered and licensed with the State of Minnesota as an RN • Minimum two years of Home Health nursing experience as an RN • Work positively and favorably with patients, families, and staff from all walks of life and experiences • Must have your own vehicle, auto insurance, and valid driver’s license • Experienced with vent, trach, EHR • Must be CPR/BLS certified • Typing WPM: minimum 40 WPM
