Licensed Practical Nurse (LPN)

The LPN delivers exceptional service by meeting client’s needs and by ensuring all health needs, the safety of all clients and personal needs are met. The LPN assists the R.N. with planning, coordinating, and managing the home care service and complies with the Comprehensive Home Care regulations. The LPN is responsible for the overall direction, coordination, and monitoring of home care service delivery under the direction of the Director of Nursing. Essential Functions (Including but not limited to the following): - Assure clients maintain residency requirements, monitor and recommend alternative placement if necessary. - Reviews client service agreements with R.N. - Monitors and maintains client charts and nursing notes. - Communicates to physicians any changes in client's needs or conditions. - Order medications, medication renewals and connect with physicians as necessary in regard to patient medication. - Fills weekly pillboxes for medication administration. - Communicates in staff communication book any information that staff needs to be aware of. - Assists with compliance in meeting regulations and is available for State Licensure Surveys. - Participate in staff meetings, committees, etc. - Assists other departments as necessary. - Report injuries for self, staff, clients or visitors immediately to RN Supervisor. - Proactively communicate and update key leaders with crucial information related to clients and personnel. - Maintain a high level of confidentiality regarding all client, personnel and vendor matters. - Document client files and store them appropriately. - Assures clients continue to live safely in their unit, monitors and recommends alternative care and services, if necessary. - Inform leadership of changes in client's needs and conditions. - Maintains communication with clients, families, and other staff. - Responds to client concerns or complaints. - Works to assure facility is in compliance with federal state and local standards and regulations. Is available for license surveys. - Maintains an up-to-date knowledge of current trends of Home Care. - Participate in on-call management rotation. Required Qualifications - Licensed with the State of Minnesota as an LPN. - Must have a valid driver’s license. Driving may be necessary. - Ability to lift/carry objects weighing up to approximately 30 lbs. or more and to meet all physical requirements related to this position. Preferred Qualifications - Experience in working with older adults. - Minimum of three years of experience in a related field.
