Will play a critical role in caring for older adults- not only physically, but emotionally, socially, mentally, and spiritually. You will also get to work with a team who has the same mission, and have the opportunity in developing and executing innovative ideas about how to best serve the older adults entrusted to our care. Responsible for the provision of high quality personal care and services to residents to support the Person-Centered approach, personally designed living choices of residents, while maintaining consistency with regulations and established best practices. Follows the client’s plan of care to help each client reach and maintain his or her highest level of performance and independence in the home environment. These services include: bathing, grooming and assisting with activities of daily living. Provides communication with client and/or family while in client’s home as appropriate. Follows schedule frequency as ordered by physician. Proactively participates in care conferences with other health team members as needed. Maintains privacy and confidentiality of client, client's records and client's living environment. Understands and follows HIPAA guidelines. Promotes personal safety and a safe environment for clients and co-workers. Other duties as assigned. Ethics and Values: Engages in open and honest dialogue with others. Will be responsible for observing, reporting, and documenting observations and patient care performed during the home care visit. Providing personal care, including baths, personal hygiene, changing bed linen, dressing/undressing, toileting activities, and light housekeeping Preparing meals and assisting in feeding patient if necessary Taking and recording oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure when ordered Assisting in ambulation and exercise according to the plan of care Performing range of motion and other simple procedures as an extensional therapy service as ordered Assisting patient in the administration of medication Assisting with patient’s laundry if needed Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.) Reporting patient’s condition and significant changes to the assigned nurse/administration Adhering to the Gonzales Adult Family Foster Care (AFFC) – 245D licensed (Loving CarePartners, LLC)'s documentation and care procedures and standards of personal and professional conduct Following all privacy policies of Gonzales AFFC-245D, Infection Control protocols, and maintaining the confidentiality of protected healthcare information (PHI) And other duties and responsibilities assigned.
