The Long-Term Care (LTC) Preparedness Toolkit was updated on February 24, 2023, and is now available on the Minnesota Department of Health website. This toolkit can help LTC facilities (both nursing facilities and assisted living facilities) prepare for the most common types of emergencies they could experience within their region. The toolkit gives guidance for specific incidents, how to respond and recover from incidents, as well as considerations to evaluate during an emergency. The toolkit also brings to light the CMS Appendix Z requirements that changed since the last update of the LTC toolkit back in 2016.

This toolkit was created with multiple partners who represent long-term care facilities and agencies. The Minnesota Department of Health worked with the Minnesota Health Care Coalitions, LeadingAge Minnesota, Care Providers of Minnesota, the Minnesota Home Care Association, and the Minnesota Network of Hospice and Palliative Care.
