MDH wants to hear from YOU!

For many years, Care Providers of Minnesota has been “urging” representatives at the Minnesota Department of Health (MDH) to provide an anonymous feedback tool for providers to use during or after a survey or investigation. There are many good reasons for this concept:
  • Provide MDH data regarding survey and investigation protocols that they can use for quality improvement initiatives
  • Provide a vehicle for providers to use to share feedback, both positive and negative, about the survey or investigation process, without the fear of retaliation
  • Provide a vehicle for providers to use to share observations about the approach surveyors and investigators use when they communicate with residents, visitors, and staff during surveys or investigations

Beginning the week of June 5, 2023, administrators and directors will be given an electronic feedback questionnaire link at the beginning and end of any survey/investigation to offer feedback on the survey/investigation experience. The link is YOUR OPPORTUNITY to provide feedback to MDH about the survey process. The electronic survey results will be anonymous. This process will be available to nursing facilities, assisted living facilities, and home care providers for both “routine” surveys and investigations.

Providers are encouraged to assist MDH with their quality improvement efforts by completing the survey during or after the conclusion of a survey or investigation—MDH wants to hear from YOU!
