AHCA/NCAL Population Health Management Summit

AHCA/NCAL’s Population Health Summit (PHM) is the only gathering of its kind specifically focused on the long-term care and post-acute care community. With CMS’s ongoing focus on shifting from fee-for-service (FFS) to accountable care arrangements, prioritizing value-based care is increasingly essential for the long-term care industry. The PHM Summit serves as a hub for innovation and thought leadership, featuring a diversity of sessions tailored to both newcomers to PHM and seasoned providers accustomed to managing risk.
