Quality reports posted to the DHS Nursing Facility Provider Portal

According to a recent communication from the Minnesota Department of Human Services (DHS), updated quality reports are now posted to the
Nursing Facility Provider Portal.
2023 quality of life resident interviews
These item-by-item reports include results of face-to-face interviews with your long-stay residents, conducted by Vital Research between October 2023 and January 2024. To view your report: click the Resident Quality of Life Ratings tab, then
2023 Observed Quality of Life Scores. DHS plans to post Risk Adjusted reports later this month.

January–December 2023 clinical QI reports:
These reports include one calendar quarter (Q4) reflecting MDS changes effective October 1, 2023. Five QIs based on Section GG Functioning items are not included in this report, as they do not yet include enough post-October 1st assessment data to produce reliable rates for scoring or quality tracking. DHS plans to include these QIs in the report ending March 31st and expects to share reports in mid-May.
Note, DHS will contact facilities individually if you are using functioning QIs for PIPP improvement projects.
To view your report: click the MN Quality Indicators tab, then select from the top dropdown menu and click View Report.
Hospitalization and community discharge reports:
The latest data ending December 31, 2023 (hospitalizations) and September 30, 2023 (discharges) is now available. To view your report: click the Hosp/Comm Discharge tab, then select from the dropdown menus and click View Report
