The Charge nurse is responsible for the provision of nursing care to residents and is an advocate for the resident. He/She is prepared by virtue of education and experience to plan and/or give nursing care to all the residents; evaluate total resident care (physical, emotional, psychological, social and spiritual); and participate with all other departments in planning and implementing the nursing component of care, and assists in the evaluation processes. She/he endorses the team concept of providing care through the various disciplines. Monitors/observes/reports work performance of Nursing Assistants and provides yearly evaluation input. QUALIFICATIONS: Is a graduate of an approved program for Licensed Practical/Vocational Nursing or Registerd Nursing Program. Has current LPN or RN licensure in Minnesota. Demonstrates leadership or potential leadership ability. Has knowledge of gerontological nursing and related aspects appropriate to the resident population. Is skilled in oral and written communication. Demonstrates the ability to collaborate effectively with other health personnel and health care agencies. In collaboration with a Case Manager, assesses the physical and psychosocial state of the residents and plans and implements nursing care. Keeps updated on new developments in geriatrics and gerontological nursing through a planned program of Continuing Education. FUNCTIONS: The LPN/RN provides total nursing care consistent with the philosophy, policies and procedures of Pierz Villa, and with the standards of Gerontological Nursing Practice. Utilizes the nursing process in his/her nursing practice which includes: 1. Monitors, observes and documents the residents’ health status and nursing needs 2. Planning the nursing actions 3. Implementing the plan of action 4. Evaluating the outcome of the action taken (which dictates reassessment and replanning). Assists in developing and implementing resident care plans through collaboration with the residents, families, and other health team workers. Assumes the role as Charge Nurse on the unit. Implements health care measures and delivers direct resident care. Works closely with other disciplines within the Villa and the professional staff involved in the resident’s care. Maintains good working relationships with Medical Staff and assumes responsibility for the execution of doctor’s orders. Makes routine rounds with the physicians. Calls physician and pursues medical needs of residents as appropriate and necessary. Cooperates in interpreting and administering the Villa’s personnel policies, Infection Control and Pharmaceutical manuals. Supports Confidentiality and is responsible for resident’s right to privacy as it relates to his medical care program, social, religious and psychological well-being. -- Provides for security of medical records, schedules, etc. on the unit. -- Is responsible for respecting and insuring the confidentiality and privacy of staff- related personal problems. Position requires full mobility and the ability to respond to emergency situations when needed. Utilizes concepts of health maintenance and rehabilitation in teaching and counseling of residents and their families. Promotes a supportive environment by utilizing communication skills and adapting the residents’ care to meet the needs of the individuals. Supports Nursing Service Policies and Procedures and uses the chain of command in communicating. Documents resident status and response to nursing actions; maintains accurate charts. Functioning as charge nurse, thenurse assumes the leadership role for resident care in an assigned area. In collaboration with the RN, the charge nurse then: Organizes, monitors, observes and reports care given by supportive personnel. Gives direct nursing care as indicated, especially to the very ill. Prepares staff assignments, utilizing knowledge of the capabilities and performance of other nursing personnel. Delegates nursing care of residents to appropriate nursing team members. Collaborates with the case manager in evaluating staff performance. Assists nursing team to learn new nursing policies, procedures, and concepts of care. Participates in the Villa FIRE AND SAFETY program. Evaluates unsafe conditions and takes corrective action as necessary. Flexibility of working hours necessary for attendance of Villa meetings as well as outside activities. Will work every other weekend. Evaluates health and resident care services. Participates in evaluation of therapeutic treatment and nursing process. Participates in resident care conferences. Must be alert to regulations set down by local, state and federal agencies and interpret and enforce as indicated. Participates in establishing and evaluating standards of nursing practice, peer review and audit mechanisms. Reviews total health care plan and drug profile and makes recommendations for change as indicated.


JOB REQUIREMENTS
MN Nursing License Long Term Care experience preferred, but not necessary.
