Foundation Scholarships

The education scholarships funded annually by the Care Providers of Minnesota Foundation are designed to assist qualified persons who wish to further their education and practice serving seniors and persons with disabilities. It is the Foundation’s intention to encourage scholarship recipients to continue to work serving seniors and persons with disabilities although this not a requirement.

In 2018, 13 scholarships totaling $17,275.00 were awarded to students in nursing, healthcare administration, physical therapy, social work, counseling & more!

2019 Timeline

May 13, 2019—Applications available
July 12, 2019—Applications due 
August 9, 2019—Scholarship recipients selected
September & October 2019—Scholarships awarded to recipients

Submission requirements

A complete application packet consists of the following:
  • Application
  • Responses to essay questions
  • Administrator/executive director/direct supervisor recommendation form
  • Personal/coworker recommendation form
It is the responsibility of the applicant to ensure the submission packet is complete and that all recommendations are received by the Foundation by the July 12, 2019, deadline. Applicants are advised to confirm both references have been completed.

If any one of these documents is missing or your information is incomplete, the application will be considered incomplete and will not be reviewed for a scholarship.

Fax submissions will not be accepted.

Applicants must notify Care Providers of Minnesota if they are no longer an employee of a Care Providers of Minnesota member.

Eligibility requirements

All applicants applying for a Care Providers of Minnesota Foundation scholarship must demonstrate the following:
  • Currently employed (full-time or part-time), working for a facility, housing, home care, or community member of Care Providers of Minnesota
    • Scholarship recipients must remain employed with qualifying Care Providers of Minnesota employer both at the time of selection & at the time of their award presentation to receive the scholarship
  • Must have at least one year of long-term care work experience
  • Enrolled (or enrolled no later than fall 2019) in an accredited course of study leading to a career path serving seniors and/or persons with disabilities
    • Scholarship recipients must show proof of enrollment before receiving funding
    • Scholarships are for current or near-future education only and NOT for past education reimbursement
  • Scholarships cannot be used for past education reimbursement or continuing education to maintain one's license

Care Providers of Minnesota Foundation Scholarship Funding

Funds for Care Providers of Minnesota Foundation scholarships come from many sources:
  • Specific scholarship funds—Hagemeyer Fund, Goldberg Fund & Lemieux Fund
  • General Foundation funds
  • Proceeds from our annual fundraising activities—golf tournament, silent auction & wall of wine
