PDPM Brochure--click to open PDF Care Providers of Minnesota is partnering with its national association, the American Health Care Association (AHCA), for a multi-faceted training initiative on PDPM.  

PDPM ACADEMY includes both in-person training & online education, with continuing updates and support.

PDPM ACADEMY is divided into three parts, and registration includes access to all phases: 
PHASE ONE: PRE-WORKSHOP WEBINARS
PHASE TWO: IN-PERSON • APRIL 24, 2019
PHASE THREE: PDPM ACADEMY REGISTRANTS WILL RECEIVE ACCESS TO MONTHLY LEARNING OPPORTUNITIES AND TOOLS

The PDPM ACADEMY workshop is an interactive learning opportunity featuring two national experts. Participants will learn from expert presenters, videos, and small group exercises.  Workshop participants will go in-depth with four modules that lay the foundation for transition planning: 

  • aligning buildings and companies to transition to PDPM using AHCA’s PDPM Readiness Tools© and the related core competencies; 
  • classifying residents in case mix groups; 
  • assessing market position in a PDPM environment; and 
  • using PDPM ACADEMY organizational, business, and management tools to reconfigure business planning.

To ensure an effective workshop experience, all PDPM ACADEMY registrants are required to complete the webinar modules before April 24, 2019. Links to the webinar modules will be emailed to all registrants.

WHO SHOULD ENROLL?
It is important for a team of staff be part of a SNF’s PDPM transition team and participate in the PDPM ACADEMY. 

PDPM ACADEMY registration is limited to Care Providers of Minnesota provider members.

Lodging
Care Providers of Minnesota has a room block at the Roseville Radisson Hotel for $109/room per night for king or queen standard. Room rate includes a hot breakfast in AXELS Charhouse. Please reserve your accommodations early and mention Care Providers of Minnesota workshop to receive this rate. Call Radisson at 1-800-333-3333. Room block cut-off date is April 1, 2019.

Cancellation/no-show policy
We understand that circumstances may arise that may prevent you from attending the April 24, 2019, workshop. However, the PDPM ACADEMY requires a specific policy. If you cancel on or before April 1, 2019, we are unable to offer any refund for cancelled registrations. On a case-by-case basis, we will consider substitutes; however, any substitutions will replace original registrants in PDPM ACADEMY. No-shows are charged the full registration fee and will receive PDPM ACADEMY materials.
